Instructions:
This tool helps assess behavioral and emotional concerns in children aged 6–18 years over the past month. Answer the questions honestly based on how often you observe these behaviors in your child.
0 = Never
1 = Sometimes
2 = Often
Each response earns points. Add up your score at the end to interpret the results.

15–22 Points: Moderate Concerns
These behaviors may be affecting your child’s social, emotional, or academic life. Consider seeking professional guidance for early intervention.
23+ Points: Significant Concerns
Your child may need immediate support to address emotional or behavioral challenges. Professional assessment and counseling are strongly recommended.
