Food Frequency Questionnaire (FFQ)

Instructions:

This questionnaire assesses how often you consume specific food groups over the past 7 days. Answer honestly and choose the option that best applies to you. Each answer earns points. Add up your total score at the end to evaluate your dietary habits.


Section 1: Fruits and Vegetables

1. How many servings of fruits (e.g., apples, bananas, mangoes, oranges) do you eat per day?
2. How many servings of vegetables (e.g., spinach, bhindi, carrots, saag) do you eat per day?

Section 2: Protein-Rich Foods

3. How often do you consume meat, fish, or chicken?
4. How often do you consume lentils (daal), beans, or chickpeas?

Section 3: Grains and Carbohydrates

5. How often do you eat roti, rice, or bread?
6. How often do you eat sugary foods (e.g., mithai, desserts, biscuits, fizzy drinks)?

Section 4: Dairy and Fats

7. How often do you consume milk, yogurt, or lassi?
8. How often do you consume fried foods (e.g., samosas, pakoras, parathas)?


Interpretation of Results:

16–24 Points: Excellent Dietary Habits

You have a balanced diet rich in fruits, vegetables, protein, and healthy grains. Keep it up!

10–15 Points: Moderate Dietary Habits

Your diet is somewhat balanced but may need improvements, such as reducing fried or sugary foods and increasing vegetables and proteins.

0–9 Points: Poor Dietary Habits

Your diet may be high in unhealthy foods or lacking essential nutrients. Professional guidance is recommended to improve your eating habits.
